Treatment options for essential tremor condition include medication, botulinum toxin injections, and surgery. Surgery is a reliable option for severely disabling tremors. Dr Gurneet Singh Sawhney states, ‘We have three procedures to treat this condition, viz Stereotactic Radio Frequency Lesioning of Ventralis Intermedius Nucleus (VIM) of the thalamus, Thalamic Ventralis intermedius Nucleus Deep Brain Stimulation (DBS), VIM Magnetic Resonance Guided Focussed Ultra Sound (MRgFUS). All three surgeries focus on providing relief from very intense and severe essential tremor in patients.’

Elaborating on the VIM deep brain stimulation (DBS), which is a reliable procedure for treating tremors experienced in this condition, Parkinson’s disease, and dystonia. The neurosurgeon implants electrodes in the VIM of the thalamus and a pulse generator under the collarbone.  The high-frequency stimulation of the VIM helps to manage the tremors. However, neurosurgeons recommend this procedure when medications are ineffective or do not suit the patient.

On the other hand, during the Stereotactic Radio Frequency Lesioning procedure, the neurosurgeon ablates VIM nucleus in the thalamus which is the source of developing essential tremors in the brain that are vital for motor control. To perform this procedure, the neurosurgeon uses detailed brain scans (MRI and CT) and a stereotactic frame, which is attached to the patient’s head to locate the exact point within the thalamus during the surgery.

VIM Magnectic Resonance-Guided Focused Ultra Sound (MRgFUS) is the latest incisionless surgery, done using ultrasound waves for VIM Nucleus ablation under the MRI guidance. This surgery is performed without making an incision or hole in the skull.

He adds, ‘These surgeries can reduce tremors in the contralateral arm and the head and voice tremors. However, an in-depth evaluation of the after-effects of these surgeries is essential before proceeding.’

He recalls the case of Stereotactic Radio Frequency Lesioning procedure on VIM Nucleus of the thalamus, which he had performed on a 74-year-old male executive with an extrovert personality. The patient had difficulty performing daily tasks because of uncontrollable tremors in both his arms. His condition worsened, and he was becoming socially secluded to avoid embarrassment. He approached Dr Gurneet Singh Sawhney as his last hope to regain normalcy.
