Dr. Vishal Khullar, a leading cardiovascular surgeon recently performed an emergency surgery to save the life of a 74-year-old male suffering from acute aortic dissection, a life-threatening condition, as there was a tear in the inner layer of the aorta, the largest blood vessel in the body.

He explains that Acute Aortic Dissection is a serious medical condition that needs emergency attention. There is a tear in the inner layer of the aorta, the body’s largest blood vessel through which blood flows between its layers; the vessel wall weakens, which can lead to rupture. In this condition, the patient complains of “tearing” or “ripping” chest pain, weakness, dizziness, shortness of breath and can become unconscious. Acute Aortic Dissection is more likely to occur in patients diagnosed with high blood pressure, atherosclerosis, connective tissue disorders, aortic valve disease and pregnancy.
Referring to this case, he states, ‘Immediate surgical intervention is the only way to treat an acute aortic dissection. The patient complains of acute chest pain radiating to the back and hence, once the condition is confirmed, surgical intervention is the only way to resolve it.’
As per the reports, the patient’s ECG showed ST-T changes in the inferior wall. A 2D echocardiogram indicated a dissection flap in the ascending aorta, a bicuspid aortic valve, and severe aortic valve regurgitation. A CT scan of the chest confirmed the presence of acute aortic dissection starting in the ascending aorta and extending to the aortic arch.
During the surgery, Dr Vishal Khullar observed that the dissection flap involved the origin of the right coronary artery and extended up to the aortic arch. Hence, he had to replace the aortic root and ascending aorta up to the aortic arch (hemiarch replacement).
As mentioned above, early diagnosis and prompt treatment play a key role in treating such patients. In such cases, cardiovascular surgeons recommend surgery to repair the damaged aorta. Other medications include medicine to lower blood pressure and heart rate.
